Group Checklist
THE FOLLOWING INFORMATION WILL ASSIST US IN OBTAINING THE MOST COMPETITIVE PROPOSALS POSSIBLE:
- Census
- Birth date, sex, type of coverage, location.
- Life insurance; we need the life volume for each insured.
- Disability insurance; we need salary and job occupation description.
- Please include COBRA participants and COBRA termination dates.
- Clear description of industry type including SIC Code, affiliates, and locations.
- Effective date
- Claims and enrollment by type of coverage (24 to 36 months, by month if possible).
- Large claims to include dollar amount, diagnosis and prognosis (half of the specific stop loss level).
- Life Insurance claims in past 3 years (number and benefits paid).
- Current insurance carrier: TPA, PPO Network(s).
- Current method of funding (fully insured or self-funding).
- If self-funded, indicate specific stop loss level, and contract basis (include aggregate contract if appropriate).
- Current rates, premium and factors.
- Percentage of contribution by employer for employee and dependent coverage.
- Copy of current schedule of benefits for each type of coverage.
